Registering

During COVID-19 you will need to either email the forms below back to us with attached photo ID and proof of address, or you can drop the forms into the box in the practice entrance.

If you are on any medication you will need to contact the surgery to arrange a telephone consultant with a GP.

If the forms aren’t filled out correctly a staff member will contact you if more information is required.

An appointment will be made for you to see the Health Care Assistant for a New Patient Check and you will be asked to bring a urine sample with you.  If you are on any medication or if you need to be seen for treatment, you will also need to see the Doctor.

We will need proof of ID to register so please bring two forms of ID including one piece of photo ID e.g. passport and a utility bill.

Language Line is used to interpret for non English speaking patients and we allocate a longer appointment to allow for this.  The reception staff have a printed form translated in several languages to assist them in identifying the language spoken.

Change of personal details

If you move or change your name or telephone number please help us by informing us so your records can be updated. Failure to do so could cause delays or prevent us from contacting you or a relative in an emergency.
